Each registrant must fill in a separate form. A clean photocopy of the form will be accepted, or additional forms can be obtained from the above link

Please note that all persons intending to attend the Conference must register, including Speakers and Chairpersons. Please read through this website carefully before you complete your form

If you are interested in ONLY attending a Pre/Post Conference workshop you can use the registration form. Please just complete your personal information details and the pre/post conference section. To advanced through sections please click on “continue” at the bottom of the screen.

The above rates (A$) are in Australian Dollars and include the Australian Goods and Services Tax (GST).

Registration Entitlements

The entitlements for each registration type are as follows:
Delegate

• Attendance at all Conference sessions
• Morning tea, lunch and afternoon tea each day (if being served)
• Welcome Reception
• Friday Drinks
• Delegate Satchel
• Conference program

Day Registration

• Morning tea, lunch and afternoon tea on the chosen day of the Conference (if being served)
• Conference program
• Delegate satchel

Payment of Fees

NOTE: Registrations will not be processed or confirmed until payment in full is received.

Within Australia
Payment can be made by cheque or credit card. Please note all transactions by credit card will appear on your statement as payment to Conference by The Meeting Planners. Cheques should be made payable to CP 2009. Your name and full address should be typed or printed clearly on the back of the cheque.

International
Payments from overseas must be made by credit card or bank cheque in Australian Dollars only, drawn on an Australian Bank and be made payable to “CP 2009”. Your name and full address should be typed or printed clearly on the back of the cheque. Fees will not be accepted by direct transfer.

Acknowledgements

Your registration and payment will be acknowledged via email or post with confirmation of your requirements according to your registration form. Your registration will not be processed or confirmed if payment does not accompany your form.

Your letter of acknowledgment will include any further advice necessary prior to your arrival at the conference.

Cancellation Policy

Cancellations received in writing at the CP 2009 Conference Organisers Office by 15 January 2009 will be accepted and all fees refunded less an AUD$100 administrative fee. Cancellations received after this date cannot be accepted and will not be refunded. No refunds will be made for non-attendance at the 3rd International Cerebral Palsy Conference.
